New patients have an opportunity to fill out paperwork online. Five days before your appointment, you will get an email to fill out the demographic and consent forms. Two days before your appointment, you will get a text message reminder and the opportunity to fill out the medical forms. If you would like to do it on paper, the forms are below. Please print out all four forms and bring them to your office visit along with a picture ID and your insurance cards. There is no way to send the forms back to us electronically so please print them out and bring them to the office visit.
